1. About these terms

These terms are an agreement between you and [registered business name] (RC/BN [number]), trading as hormones.ng ("we", "us"). By taking a quiz, paying for a plan or using our service, you agree to these terms and to our privacy policy. If you do not agree, please do not use the service.

2. What we offer

• Free online health quizzes, which are a screening tool, not a diagnosis.
• Paid doctor reviews of your answers, photos and results, with a written plan sent on WhatsApp.
• Phone or video calls, if your plan includes them.
• Lab test requests and prescriptions, when your doctor decides they are right for you.
• Lab results explained, and second opinions on a diagnosis or treatment plan.

Online care has limits. A doctor cannot examine you in person. If your doctor thinks you need a physical exam, a scan, a procedure or hospital care, they will tell you, and may refer you elsewhere.

3. Not for emergencies

Do not use hormones.ng for emergencies. If you have chest pain, trouble breathing, sudden weakness, sudden vision loss, a severe headache, heavy bleeding, a severe allergic reaction, or thoughts of harming yourself, go to the nearest hospital now. Our reply times are not fast enough for emergencies.

4. Who can use the service

• You must be 18 or older.
• The quiz must be about you, and answered by you, unless you are helping someone who agrees and is present.
• Someone else, such as a family member abroad, may pay for you. The care agreement is still with you, the patient.

5. Our doctors

Your care is given by American and Canadian trained doctors [licensed by the Medical and Dental Council of Nigeria; registration number(s): …]. Your doctor uses their own clinical judgement. This means your doctor may:

• ask you more questions, or ask for tests or photos;
• decide that a treatment you asked for is not safe or right for you;
• decline to prescribe, or advise in-person care instead.

Paying for a plan pays for a medical review. It does not guarantee a particular diagnosis, prescription or result.

6. Your responsibilities

• Give true and complete answers, including all medicines, supplements, allergies, health conditions, and whether you are or could be pregnant.
• Upload only your own photos and records, or ones you have permission to share.
• Follow your doctor's plan, read medicine labels, and tell the pharmacist about other medicines you take.
• Tell us about side effects or new symptoms. Get urgent care if they are serious.
• Keep your phone and WhatsApp secure, because we send your plan there.
• Treat our team with respect. We may end care if a patient is abusive.

7. Prescriptions and medicines

• We do not sell, stock or deliver medicines.
• If your doctor prescribes treatment, you receive a written prescription. You may fill it at any licensed pharmacy you choose, and you pay the pharmacy directly.
• The pharmacy is responsible for the medicine it gives you. Ask for genuine, NAFDAC-registered products and check expiry dates.
• Do not change, copy or reuse a prescription. Each prescription is for you only.

8. Lab tests

Lab tests are done by independent labs that you choose. You pay the lab directly. The lab is responsible for collecting samples and the accuracy of its results. When you send us results, we review them as part of your plan.

9. Lab results explained and second opinions

These reviews are based only on the results, records and answers you give us. If something is missing, unclear or old, our advice may be limited. A second opinion is advice for you to discuss with your treating doctor. It does not replace your treating doctor unless you choose to move your care to us.

10. Plans, fees and payment

• All prices are shown in naira before you pay. A US dollar amount, if shown, is an estimate only.
• You can pay online through Paystack or by bank transfer. Your review starts once we confirm payment.
• Reply times (for example "within 24 hours") start when payment is confirmed. They are our target, and we work hard to meet them. [Lawyer: decide whether reply times count weekends and public holidays.]
• VIP (Executive) places are limited each day. Joining the VIP waiting list does not guarantee a place.
• Medicines and lab tests are not included in plan fees.
• We may change our prices. The price you paid is the price for that review.

11. No refunds

Your fee pays for your doctor's time to read your answers, photos and results. For this reason, fees are not refundable once your review has started. This includes when your doctor:

• decides a treatment is not safe for you, or declines to prescribe;
• asks you to do tests or see a doctor in person;
• gives a diagnosis you did not expect.

We will return your money only if you paid twice by mistake, or if we are unable to provide the review you paid for at all. [Lawyer: check this against the Federal Competition and Consumer Protection Act 2018.]

Information you give us

By using hormones.ng, including the free quiz and free health report, you agree that we save the information you enter and use it as described in our privacy policy: to care for you, to contact you, and, with names removed, to improve our service. We never sell your information.

The free health report

The free health report shows your symptom score, checks your answers for warning signs, and gives general self-care tips. It is a screening tool. It is not a diagnosis, treatment plan or prescription, and it does not create a doctor–patient consultation. If it flags warning signs, our team may contact you on WhatsApp. If you feel very unwell, go to the nearest hospital.

12. WhatsApp, calls and messages

We send plans and messages by WhatsApp, and may call you. WhatsApp is run by Meta, not by us. If you would like a different way to receive your plan, tell us. Calls may be noted in your medical record. We do not record calls without telling you first.

13. No guaranteed results

Every body is different. We cannot promise that a treatment will work, or how fast. Stories and reviews on our website are real patients' experiences and are not a promise of the same result for you.

14. Our responsibility to you

We will provide our service with reasonable care and skill. To the extent the law allows, we are not responsible for: loss caused by false or missing information you gave us; the medicines, products or services of pharmacies, labs or hospitals we do not run; or problems caused by internet, phone or WhatsApp outages. To the extent the law allows, our total responsibility to you for any claim is limited to the fee you paid for the review in question. Nothing in these terms limits responsibility that cannot be limited by Nigerian law, including for death or personal injury caused by negligence. [Lawyer: review this clause.]

15. Ending care

You can stop using our service at any time. We may stop providing care if you give false information on purpose, misuse a prescription, or are abusive to our team. If we end care, we will tell you why where we can, and you can ask for a copy of your records.

16. Website content

The website, quizzes and content belong to us. You may use them for your own personal care. You may not copy, sell or reuse them without our written permission. Health information on the website is general and is not personal medical advice until a doctor reviews your case.

17. Law and disputes

These terms are governed by the laws of the Federal Republic of Nigeria. If you have a complaint, please email hello@hormones.ng first, and we will try to solve it quickly. If we cannot agree, either of us may refer the dispute to mediation at [the Lagos Multi-Door Courthouse] before going to court. The courts of [Lagos State] will have jurisdiction.

18. Changes

We may update these terms. The date at the top shows the latest version. The terms in force when you paid apply to that review.
